MEDICAID SERVICE COORDINATION (MSC)

This program is an individual/consumer driven service designed to maximize home and community support to the mentally retarded and developmentally disabled. The goal of MSC is to maintain and ensure quality service connections and support services for all consumers through service coordination. Service coordination is the comprehensive and professional assistance with self management of all parts of an individuals life.

There are four components to this program:
Intake/Assessment/Interview - determines the individual's eligibility.
Service Planning and Coordination - determines the person's service needs and coordinates services for them.
Linkage/Referral - helps the individual to access programs and services.
Follow Up and Monitoring - ensures the individual's benefits from the program and services.


REQUIREMENTS



Must have a developmental disability; i.e. mental retardation, autism, down syndrome, pervasive developmental disorder, and/or cerebral palsy.



Must have Medicaid and reside in Nassau County.
