AT HOME RESIDENTIAL HABILITATION (Res Hab)

This service is tailored to provide at home teaching and skill building/development for a developmentally disabled individual. It includes the acquisition, retention, and improvement in skills related to life, safety, activities of daily living, and social and adaptive skills. These skills are necessary for the individual with a disability to remain in their home setting.

REQUIREMENTS



Must have a developmental disability; i.e. mental retardation, autism, down syndrome, pervasive developmental disorder, cerebral palsy, etc.
Must have Medicaid and reside in Nassau County.
